Because of our new involvement with the Northwest Pharmacy Convention, ASCP President Hedva Levy, will be visiting Oregon as part of her trip to the convention.OSPA, OSCP and Acadia are offering you the opportunity for a free dinner meeting and CE Presentation by Dr. Levy.  All pharmacists are welcome and you do not need to join the Senior Care Committee to attend the presentation.

Education course title: Challenges and Opportunities of a Home-based Consultant Pharmacy Practice for Seniors
In this presentation Dr. Levy will review how she developed her home-based consultant pharmacy practice. She recognized that many residents of nursing homes and assisted living facilities would have been able to remain in their homes were it not for the difficulties they experienced managing their medications and dealing with medication related problems. Dr. Levy will review the steps necessary to establish such a pharmacy practice and will discuss the challenges as well as professional opportunities offered by such a practice.

Learning Objectives:
After attending this presentation the participant will be able to:

  1. Identify three steps involved in establishing a home-based consultant pharmacy practice for seniors.
  2. List three barriers to establishing a home-based consultant pharmacy practice.
  3. Discus how a home-based consultant pharmacy practice will advance the pharmacy profession and improve patient outcomes for seniors.
